Job summary
We are seeking to recruit a Neurorehabilitation Case Manager to work with a range of stakeholders to improve Specialised Rehabilitation services for patients with highly complex needs across the Midlands.
Main duties of the job
The postholder will need to have an excellent understanding of the specialised and core services that currently support this population and have a strategic vision for future requirements. They will be expected to work in partnership with a wide range of patients, carers and stakeholders and will be responsible for reducing delays in the patient journey associated with admission to and discharge from Specialised Rehabilitation Services.

Successful candidates will have a proven track record of performing using high levels of autonomy. We are looking for self-starters with specialist knowledge of the therapeutic and rehabilitation requirements for patients with neurological conditions, specifically sudden onset conditions. We are also looking for candidates with strong leadership skills and the ability to work closely with multi-agency partners to support some of the most complex cases in the service. These individuals will have a specific responsibility for driving transformation within Specialist Rehabilitation Services within the Midlands; they will also provide support to fellow Case Managers at local, regional, and national level.
