Job summary
The Urgent and Emergency Care (UEC) & Virtual Care (VC) Improvement Team are subject matter experts and improvement practitioners based within the Operations Directorate in the East of England (EoE) Region. The team works closely with regional UEC Operations and Performance teams, providing expert support and challenge to ensure delivery of regionalUEC & VC Improvement Teamobjectives. The team works directly with the six Integrated Care Systems (ICSs), and indirectly with the 14 acute providers, 11 Community Providers, EoE Ambulance Service and Primary and Community Care Urgent Care Services across EoE.
The Team aims to:
Improve quality, resilience, and operational performance ofUEC & VC Improvement Teamservices,
Identifyand address local population levelhealth inequalities and unwarranted clinicalvariation,
Improve quality of ourservices by reducing avoidable emergency admissions & facilitating earlier discharges,
Supportdelivery of Integrated Care Board, Regional and National Improvement Urgent and Emergency Care priorities.
The Team adopts the following principles in all its work;
Patient focused and clinically led,
Whole system approach in working with ICSs and key partners, adopting a population health approach,
Peer challenge and support to speed up adoption of innovation/best practice,
Usingdata and evidence to drive sustainable improvements for patients.
Main duties of the job
You will employ matrix working to deliver workstreams and support systems to deliver their improvement priorities.
You will be responsible for managing projects from concept through to mobilisation working collaboratively with colleagues across whole urgent and emergency care systems.
You will work with systems to support service improvement and work in partnership to build-on improvement initiatives that better meet patient needs.
You will work with a network of stakeholders across Integrated Care Boards, region and national, keeping up to date with best practice and latest developments.
It is essential the postholder is easily able to travel to the relevant Integrated Care Boards and key providers as and when required. There is also an expectation of attendance at monthly, face to face meetings at Victoria house, Cambridge.
