Job summary
As the AACC development associate (delivery lead) within NHS England's Nursing Directorate, you will have knowledge and management experience of Continuing Healthcare policy and practice and have an understanding of the CYPCC framework, joint funding and FNC.
The post holder will be required to work closely and build relationships with a wide range of NHS CHC and AACC professionals, both at a national and local level, and lead the delivery of the sustainability workstreams working closely with the RDU and ICB leads.
Main duties of the job
To Lead on the workstreams from the Recovery and Sustainability board.
To Support systems to be complaint with digital requirements, Continuing Healthcare Assurance Tool (CHAT) and Patient Level Data Set (PLDS) reporting.
Towork with the national and regional All Age Continuing Care (AACC) teams, regional NHS AACC leads and NHS AACC Teams to ensurethat the projects and deliverables meet sustainability targets whilst maintaining quality of care.
