We are seeking a compassionate, motivated, and skilled Mental Health Nurse to join our multidisciplinary team within the Inclusion Drug & Alcohol Service, working specifically with individuals experiencing homelessness.
You will play a pivotal role in supporting people with co occurring mental health conditions and substance use needs, helping to break down barriers to engagement and improving health outcomes for some of the most vulnerable members of our community.
This is a varied and rewarding role where no two days are the same. You'll work in partnership with local homelessness services, hostels, outreach teams, primary care, and wider mental health services to provide safe, person centred care.
Main duties of the job
o Provide mental health assessments, risk assessments, and care planning for people with complex needs.o Work proactively with individuals who may be non-engaging, transient or have experienced trauma.o Deliver evidence based interventions, including harm reduction advice and relapse prevention support.o Collaborate with substance misuse workers, social care, homeless outreach teams, and health partners.o Support individuals to access physical health care, safeguarding support, housing, and wider community services.o Contribute to the development of trauma-informed, psychologically safe pathways for people experiencing homelessness.o Maintain clinical records, care plans, and documentation in line with professional and organisational standards.
