Job summary
To provide a dedicated comprehensive healthcare function to the Clinical team to support improved health care for people who use drugs and alcohol and their families and friends within the organisational framework of Telford STARS
Responsibilities include:
- Provision of substance misuse treatment within Princess Royal Hospital Telford and community to Service Users within the locality.
- Working to reduce unplanned substance misuse related admissions and attendances to the Hospital setting.
- Supporting the levelling up of health inequalities agenda by supporting health checks and accessing other supports for the services
- Holding a small caseload of service users and being responsible for offering/accessing a range of interventions to that individual
- Supporting a service user through their treatment journey from beginning to end within Hospital settings.
- Embracing the concept of recovery and demonstrating this in your clinical work
- Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation.
- Keeping up to date and accurate records and documentation.
Main duties of the job
You will be responsible for managing a defined caseload of substance misuse patients, according to care plans agreed with the service user, carers and responsible Clinician, conducting baseline health checks for all new referrals, assessing need and making appropriate recommendations as part of the healthcare team.
You will provide a range of interventions for substance misuse clients and clinical assessment within Princess Royal Hospital Telford and A&E and provide harm reduction advice, information and support with an emphasis on health improvements, safer drug use, safer sex, HIV, Hepatitis A,B and C and other blood borne infections You must be flexible in seeing service users in a variety of community venues as well as making home visits consistent with need.
