Job summary

  • The direct provision of high quality evidence based person-centred sexual health care to patients within the HIV and sexual health pathways in the outreach setting.
  • To provide a sexual health service to individuals and/or groups in a community setting. The role will include a range of activities, as per the service spec, addressing the individual health and well-being, the development of pathways into specialist services, training of community partner staff to manage clients out of hours, and improve the overall health inequalities via;
  • Health promotion/prevention
  • General Sexual Health screening
  • Engagement in Primary Care services and facilitate access to organised health care

The collective aim of the Public Health service is to help those with a range of health needs including sexual health, drug and alcohol to access the appropriate treatment, and to address the physical and mental health of vulnerable and hard to reach communities.

Furthermore, the aim of the service will be to reduce health inequalities and improve pathways with community services and Sexual Health and HIV services. The service should increase access to preventative and non-emergency/emergency sexual health care; this will include, contraception, general health promotion, STI screening, treatment and liaising with the health advising team as necessary.

Main duties of the job

  • The direct provision of high quality evidence based person-centred community sexual health care to clients within Staffordshire, Shropshire and Telford.
  • Includes delivery of clinical integrated sexual health care and a wide range of universal and targeted clinical prevention, education and support initiatives in various community venues across Staffordshire, Shropshire, Telford and Stoke-on-Trent.

  • Providing and supporting an effective, efficient and high quality Public Health, Prevention and Health Adviser service that is responsive to the needs of all patients, and services within the Sexual Health & HIV Services.
  • Contributing data analysis and reporting compliance levels and data issues to meet the aims and objectives of the quality and performance agenda of the Sexual Health & HIV Services.
  • To provide clinical direction, training and support to the Prevention and Health Adviser team, whilst ensuring effective working relationships and efficient practices.

Job responsibilities

Outreach and Prevention to Staffordshire, Shropshire, Telford and Stoke-on-Trent

  • Contribute to a sustainable model of prevention with a strong focus on ensuring engagement with those individuals in the population most at risk of sexual ill-health, to promote awareness and accessibility of the service.
  • Support the delivery of a dedicated annual Prevention and Outreach plan
  • Working in partnership with other providers to engage and support vulnerable and hard to reach people through a make every contact count (MECC) approach.

Ensure that information and advice on all aspects of sexual health and contraception are consistent and easily accessible for service users online.
  • Work with the wider prevention team on the design and delivery of health promotion activities targeting MSM with the aim to prevent the transmission of STIs and HIV by encouraging condom use, promoting safer sex and regular testing and screening.
  • Support the coordination across Staffordshire, Stoke-on-Trent, Shropshire and Telford of a unified model for the C-Card Plus scheme (as per service spec) to embed condom distribution into wider sexual and reproductive health delivery outlets to include;

  • Groups such as youth offending teams or looked after children's teams.
  • Acknowledge and be aware of the specific sexual health needs of those most at risk of poor sexual health and chaotic lives, ensure services are designed to target these identified groups, as per the service spec.
  • Promote contraception and sexual health services through prevention, diagnosis and treatment and the promotion and supply of contraception in community settings
  • Ensure outreach sexual health services are easily accessible for young people at times and locations that best suit their need.
  • Ensure that a holistic view is taken of a young persons needs and possible risks. Ensure referral pathways are in place where any concerns around factors not directly relating sexual health such as substance misuse, mental health or exploitation of any kind need to be referred to relevant partners for their assessment or for advice and guidance
  • Ensure Prevention services adapt to any changes to the local landscape of sex work and continue to meet the needs of this group accordingly.
