Main duties of the job

  • To work as part of a multi-agency team delivering interventions to safeguard adults and/or children at risk.
  • To work with families to minimise and prevent, as far as possible, children entering care due to substance misuse and other related issues.
  • To engage and undertake comprehensive needs led assessments concerning substance misuse in order to reduce risk and maximise safety within the family setting.
  • To support individuals in accessing and engaging with the local substance misuse team.
  • To raise awareness around the dynamics of both substance misuse and Recovery and the potential impacts on self and on children.
  • To provide information and advice to colleagues within the Family Safeguard Team regarding the risk and protective factors surrounding families experiencing substance misuse and recovery.
  • Identify and deliver effective interventions for substance misuse to increase safety and reduce risk.

KEY DUTIES AND RESPONSIBILITIES:

Client Work

  • To deliver a range of interventions to families and carers through structured and unstructured 1-2-1 and group interventions

  • To undertake ongoing assessment of substance misuse, individual and whole family needs. For assessment to include assets, potentials & strengths as well as risks, concerns & specific dangers relating to safeguarding issues.

Where needed to effectively link families with structured services and recovery opportunities for substance misuse

  • To promote Recovery opportunities and link families and carers with Peer support and mutual aid as appropriate

  • Monitor, review and reassess Recovery plans and Risk Management plans with clients.

To provide appropriate leaflets, information and promotional material to clients.

  • Plan, develop and oversee interventions tailored to meet identified needs and agreed goals within the overall support package.

  • Keep records of all work, produce reports as required and contribute, to the monitoring and evaluation of the FIT project.

  • Make an assessment of social care needs of any carers, including identifying young carers

  • Devise individual care plans with the person, which will be part of the family care plan, based on assessed needs

  • Provide 1:1 support to enable the person to understand and deal with their substance misuse and its impacts on their children and other family members.

  • Provide brief interventions to enable the person to understand and deal with their substance misuse and its impact on their children and other family members.

  • Provide group-work to enable the person to understand and deal with their substance misuse and its impact on their children and other family members.

  • Identify and link with HCC to procure appropriate rehabilitation care packages for eligible individuals

  • Safeguard adults and children at risk

  • Refer individuals to other appropriate services, with the necessary consent.

Liaison

  • Promote partnership work and develop integrated care pathways with local family focused agencies, including Social Care, Young Carers, Carers services, LSCB, Family support Groups

  • To develop and sustain links with multi-disciplinary services including statutory and non statutory providers of Tier 2,3,4 drug services, safeguarding services, criminal justice services and providers of wrap around services.

To deliver specialist training and educational sessions to a range of relevant professionals.

Update and develop pathways and protocols specific to the role in conjunction with the management team.

  • To attend relevant meetings to represent Inclusion and the FIT team when required.

  • To act as a lead link between local hub and FIT teams ensuring up to date communication is maintained and use of resources is maximized.

Administration

  • To keep up to date records and information for the purpose of internal and external evaluation.
  • To make appropriate use of substance misuse and childrens social care database and electronic case management systems.
  • To provide information and statistics consistent with the Drug Action Team (DAT), FIT Team and Inclusions requirements.

General

  • To participate in regular supervision.
  • To take part in annual appraisal and review based on the NHS knowledge and Skills Framework (KSF)
  • To attend relevant internal and external meetings.
  • To be prepared to work some evenings, weekends and bank holidays.
  • To demonstrate commitment to evidenced based practice and keep up to date with trends and changes in the alcohol & drug field.
  • A commitment to the equal opportunities and confidentiality policies of SSSFT & HCC.
