Job summary
Inclusion services at HMP Hewell are embarking on an exciting new development to our service model to ensure a more comprehensive and patient centred approach to our vulnerable service users.
HMP Hewell as a prison service has developed an innovative way of supporting their prisoners in collaboration with Health care provisions.
The exciting new model encompasses the Quality Network standards in a structured, safe and comprehensive manner. The service will be following three pathways of care.
Early days will provide the service users with a comprehensive triage and introduction to prison and establishing their level of need. As part of the Early days model there will be introduction and wellbeing groups to empower service users to manage their concerns where deemed appropriate.
Planned care will offer planned intervention's, such as group work and individual interventions, medicines management and psychological input
Unplanned care will support individuals in crisis who require high levels of support and intervention's.
We pride ourselves in a very good collaborative working with Primary care who are supplied by Practise Plus Group and essentially allied working with the prison service.
We have recently had a HMIP/CQC inspection- though we have not had the official feedback the preliminary feedback was extremely positive.

4 full time positions (37.5 hours per week) and 1 part time (18.75 hours per week).
Main duties of the job
Provision of mental health interventions to Service Users within the prison establishment across wellbeing, primary and secondary mental health.
Early days will be assessment and establishing level of need and signposting to relevant support and offering Wellbeing groups, developing service user involvementPlanned care post holder will being responsible for managing own caseload, with regular caseload supervision, care planning, delivering interventions and reviewing care of patients located within the prison with a mental illness, within the framework of the multi-disciplinary team and Care Programme Approach. Providing Group work with support from Psychology team and developing service user involvement.Unplanned care will require to support services users through crisis period, including risk management and working closely with prison service and primary care.Post holders are expected to work as an integral member of the multi-disciplinary, multi-agency team to provide advice and support to primary health care based within the prison and to liaise with, and provide advice and support to statutory and non-statutory agencies.
