Our Integrated Mental Health and Substance Misuse Service at HMP Oakwood is looking for dynamic and experienced RMNs to join our busy, friendly and professional team.The Integrated Mental Health & Substance Misuse Team is an innovative forward thinking team, committed to a Recovery focused approach that meets individual's needs.
This is an exciting and energetic role for an individual who is passionate about providing good quality mental health care and support within a busy and rewarding environment. The variety of the role will keep things fresh and stimulating whilst being part of a supportive team with effective management providing supervision and guidance.
Main duties of the job
You will be responsible for the assessment, planning, implementation and evaluation of evidenced based interventions. The postholder will manage a caseload of service users, practicing within the framework of the multi-disciplinary team and Care Programme Approach. The postholder will provide a care coordinator role and communicate effectively with other professionals internal and external to the Inclusion team and prison to ensure throughcare support where appropriate.
You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway.
The postholder will benefit from the experience and support of the team and that of its NHS employer. Inclusion (part of Midlands Partnership NHS Foundation Trust) has prison substance misuse and mental health teams across the country and you will benefit from training and development opportunities as well as the support of an NHS employer experienced in providing prison healthcare.
