Job summary

Poised between Primary and Secondary Care, the Primary Care Gateway Services team in Digital Health & Care Wales (DHCW) play a vital role in ensuring reliable, safe communications between the two domains:

Referrals for treatment or investigation are taken from the GP systems in their Practices and transmitted to local hospitals for prioritisation and scheduling of appointments. Updates are fed back to the Practice, allowing the patient's electronic record to be kept up to date. Details on medications can also be shared between GP Practice and hospital as required, to ensure consistent safe care.

Similarly, the GP Practice can request diagnostic tests (such as blood tests) electronically, and transmitted to the hospital laboratories, allowing linking to the specimens via barcodes, saving time and reducing potential errors. Once complete, the test result is returned to the Practice for incorporation into the patient's electronic record.

We are looking for 2 Senior Support & Business Analyst to support the delivery of these services as well as playing a key role in the support of the future service expansions.
