Job summary

We are actively seeking motivated and passionate Health and wellbeing coaches for our new integrated health program across the East Riding of Yorkshire.

The contract involves delivering tier 2 smoking cessation, adult weight management, and family weight management programs, as well as providing brief interventions for low-level mental health, alcohol misuse, and overall health and well-being.

As a Health and wellbeing coach, youll offer high-quality interventions to service users aged 12 years and above. Our delivery model combines one-on-one and small group sessions.

Main duties of the job

  • Deliver holistic assessments either via telephone, video call/e-mail or in person, as requested by the service user
  • Deliver weight management and smoking cessation programmes to service users aged 12 and above through one to one and group-based models, either face to face or remotely
  • Make onward referrals and signpost service users to external local and national specialist support agencies
  • Attend community events and promote the service in a professional and knowledgeable manner
  • Work closely with service users throughout the programme delivering a first class service and knowledge
  • Provide guidance whilst taking service users on their journey throughout the programme
  • Maintain data monitoring records of all service users and ensure that this is handled with the greatest care
  • Consistently check and monitor the progress of the service users
