Job summary

As a diabetes practitioner, your role involves coaching service users in the type 2 diabetes remission program. Through one-on-one face-to-face sessions, youll encourage behavioral changes, help set healthy lifestyle goals, and support users in completing the program, overcoming any barriers they may face.

Main duties of the job

Every day you will...

  • Work closely with service users throughout the programme delivering a first class service and knowledge
  • Conduct 1-1 coaching sessions to service users across your allocated areas, often taking measurements such as weight, blood glucose and blood pressure.
  • Maintain data monitoring records of all service users and ensure that this is handled with the greatest care
  • Support the service user engagement strategy to ensure adherence to the program and minimise drop-outs

The Diabetes Practitioner This will be to support the delivery for the LCD (Low Calorie Diet) through F2F methods

A Key focus on the role will be to encourage and support patients to complete the programme including to overcome potential barriers that they are facing.

For the LCD Programme, the Diabetes Practitioner will be focused on Patient interaction and will demonstrate an empathetic and motivational approach to better support the Patients.

Should the need arise, the Diabetes Practitioner may also be required to support other Services

Delivering value to the business:

Putting service users health and wellbeing first

Supporting the NHS in making local communities healthier

Tailoring to service users needs
