Are you a skilled and motivated Advanced Nurse Practitioner looking to make a real impact in primary care?

Gateshead Inner West PCN is seeking an ANP to join our dynamic and supportive multidisciplinary team. You will work both autonomously and collaboratively to deliver high-quality, comprehensive care to a diverse patient population.

In this role, you will undertake clinical assessments, diagnosis, treatment, and management of both acute and chronic health conditions. You will focus on delivering person-centred care, tailoring treatments and support to meet the unique needs of each patient.

This role offers an exciting opportunity to work within a progressive primary care network dedicated to improving patient outcomes through compassionate, evidence-based practice.

This post is offered as a part-time position - 15 hours per week, covering evening and weekend shifts, offering flexibility while playing a key role in supporting our Enhanced Access services.

Main duties of the job

The Advanced Nurse Practitioner will play a crucial role in delivering our Enhanced Access services during evening and weekend clinics. As part of a multidisciplinary team, you will take the lead in providing timely, high-quality clinical care through thorough assessment, diagnosis, treatment, and management of patients.

Key responsibilities include:

  • Performing detailed clinical assessments and physical examinations

  • Diagnosing and managing both acute and chronic health conditions

  • Independently prescribing medications within your professional scope

  • Ordering, interpreting, and acting on diagnostic tests and investigations

  • Leading reviews for long-term conditions and creating tailored care plans

  • Delivering patient education and promoting health and wellbeing using evidence-based approaches

  • Collaborating closely with other healthcare professionals to ensure coordinated and seamless patient care

Based in the Inner West of Gateshead our Primary Care Network serves over 45,000 patients across our five member Practices of: Whickham Medical Cottage, Chainbridge Medical Partnership, Glenpark Medical Practice, Teams Medical Practice & Sunniside Surgery.

We provide integrated services to our local population. Aiming to deliver more personalised, coordinated health and social care for people close to home.

The successful candidate will work alongside our newly established enhanced access team across all five sites.
