We are looking for a compassionate, enthusiastic and caring Healthcare Support Workers, who are reliable and flexible to join our team.

Main duties of the job

The Healthcare Support Worker will provide high quality and compassionate care to individuals. They will have the understanding and ability to deliver delegated care under the direction of their supervisor, and support the multidisciplinary team in the delivery of high quality care to enable individuals to achieve their person-centred goals in their own home.

The post holder will carry out well-defined routine duties in accordance with the individual's care plan, promote independence and check on the individual's overall progress, comfort and wellbeing.

When working as a lone worker the post holder will act on own initiative to make a situation safe and report any concerns as required.

About us

Kingswinford & Wordsley Primary Care Network is a network of 5 practices with a list size of over 50,000 patients. This role exists to support the delivery of the Kingswinford & Wordsley Primary Care Network agenda. The post will form an integral part of the PCN leadership team working alongside the Practice Managers and the Clinical Director and the PCNPartners.

Job responsibilities

Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions.

Plays a key role in vaccination and immunisation programmes within the PCN.

Works within a Multi-Disciplinary Team to support patients with long-term conditions and health inequalities.

Provides clinical support to registered nurses and other healthcare professionals within the PCN.

Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives.

  • Phlebotomy / venepuncture certification
  • Maths Qualification
  • English Qualification

Experience

Essential

  • Experience of working in a General Practice.
  • NHS Health checks.
  • Use of IT systems including EMIS, Care Notes, Microsoft office, email and database systems.
  • Preparing and maintaining environments and equipment before, during and after patient care.
  • Sterilising, cleansing and maintenance of surgical equipment.
  • Covid vaccine re-stocking and stock rotation.
  • Work closely with PCN Care Coordinator.
  • Has relevant experience in health promotion, prevention, and supporting long-term condition management;
  • Works under the supervision of a registered clinician while developing
  • competencies in community-based care; and
  • Is working at Support Worker Level Practice as described in the Primary
  • Care and General Practice Nursing Career and Core Capabilities Framework.
