5. JOB PURPOSE/SUMMARY
CBC Workforce Solutions are developing a
Flexible Workforce Hub and are looking to recruit experienced, Registered
Practice Nurses. Practice Nurses will provide quality clinical care and manage long-term
conditions, supporting our member practices throughout Gateshead.
You can choose shifts from across Gatesheads
Primary Care Network via our online rota system.
We believe in recruiting locally, to build
stronger relationships with our partners and patients. We have a dedicated team
based in Team Valley who will support applicants with the application and any
training required.
6. KNOWLEDGE,
SKILLS, AND EXPERIENCE REQUIRED

Registered as a General Nurse with NMC
-
Nursing experience within General Practice
and/or Primary Care
-
Excellent verbal and written communication skills
-
Ability to work as part of a team

Excellent organisation and time management
-
Experience managing long term conditions e.g.,
asthma, COPD, diabetes, CHD
7. MAIN DUTIES AND RESPONSIBILITIES
-
Clinical care interventions, medications,
assessment, wound care and management, cervical screening, ear care, vaccines,
pre-travel health, prescribed therapies
-
Health promotion alcohol screening, smoking
cessation, healthy diet, physical activity, weight management, sexual health and
family planning
-
Sexual health health promotion, investigation
and contraception
-
Long term conditions screening, diagnosis,
ongoing management, and referral to other agencies in line with NICE guidance
and Quality and Outcome Framework indicators
-
Develop and follow referral pathways
-
Risk stratification and care planning
-
Quality assurance and service support
-
Recognise opportunities to reduce inequalities
and unwarranted variation
-
Handle sensitive and confidential information
-
Ensuring the privacy, dignity and safety of
individuals is maintained at all times

Use of clinical IT systems, and maintaining
accurate patient records
-
Recognise and report issues relating to
safeguarding vulnerable children and adults
8. CONFIDENTIALITY
In the course of their work the post-holder
may have access to confidential information relating to the company, its staff,
stakeholders, and patients. All such
information must be treated as highly confidential, and any breach will be
subject to disciplinary action. In addition,
the nature of this role may mean that the post holder also has access to
information regarding local general practices and this should also be treated
as confidential.

10. PERSONAL
AND PEOPLE DEVELOPMENT
The
post-holder will participate in any training implemented by CBC Workforce
Solutions as part of their employment. The
post holder may be required to assist in recruitment and retention of staff
within the organisation as and when required.
The post holder will support junior or trainees.

12. HEALTH
