Working Patterns:32.5h per week

Competitive salary: DHU Band 8b.0 £59,226 - £69,066

• What your typical day looks like:
• To lead the pharmacists in delivering therapeutic interventions for chronic disease management within the practice.
• Provide oversight and assurance of acute prescription requests within the surgeries.
• Maintain the medicines reconciliation on transfer of care.
• Provide expertise in clinical medicines advice to the team of practice-based pharmacists addressing both public and social care needs of patients in the GP practice.
• Provide clinical leadership on medicines optimisation and quality improvement and manage aspects of the quality and outcomes framework and enhanced services.
• Work within the regulatory requirements, codes and Professional Standards of practice of the GPhC.
• To maintain professional and clinical competence through mechanisms of continuing professional development.

To be a successful GP Surgery Lead Pharmacist youll need:

Completion of an undergraduate degree in Pharmacy.

Pharmacist registered with the General Pharmaceutical Council (GPhC).

Basic Life Support and Child and Adult Safeguarding

At least 2 years experience as a pharmacist, demonstrated within a practice portfolio.

Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.

Experience of undertaking clinical medication reviews

Experience of clinical audits of prescribing

Experience of using SystmOne

Hold an independent prescribing qualification.

Qualifications

Essential

• Completion of an undergraduate degree in Pharmacy.
• Pharmacist registered with the General Pharmaceutical Council (GPhC).
• Basic Life Support and Child and Adult Safeguarding

Desirable

• Specialist knowledge acquired through post-graduate diploma level of equivalent training/experience.

Experience

Essential

• At least 2 years experience as a pharmacist, demonstrated within a practice portfolio.
• Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
• Experience of undertaking clinical medication reviews
• Experience of clinical audits of prescribing
• Experience of using SystmOne
• Hold an independent prescribing qualification.

Desirable

• In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.

Skills & Knowledge

Essential

• Recognises priorities when problem-solving.
• Able to identify deviations from a normal pattern.
• Able to critically evaluate and review literature.
• An appreciation of the nature of GPs and general practices.
• An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
• Able to obtain and analyse complex technical information.
• Able to produce timely and informative reports.

Desirable

• Full driving license.
