Job summary

Cluster Business Implementation and Development Manager

An exciting new fixed term position for one year has arisen for an enthusiastic and suitably qualified individual to join the innovative and forward thinking team. We aim to improve health outcomes for about 56,000 patients.

To help us achieve this, we are looking for an individual who can work co-operatively and collaboratively across all Cluster members. The post-holder will work closely with the Cluster Lead and act as a point of contact for Cluster members. The role will have a distinct focus on supporting the development and implementation of Cluster initiatives, with a strong emphasis on monitoring and evaluating outcomes and generally accelerating progress.

You should be able to provide essential support in developing and implementing the Cluster’s Integrated Medium Term Plan (IMTP) taking a lead role for specific improvement projects. The successful candidate will work closely with stakeholders to identify and act upon opportunities for service modernisation, providing information, updates and advice to support the review of delivery against the Cluster’s key priorities as outlined in the IMTP.

The successful candidate will be employed by the GP Practice/Medical Centre, not Swansea Bay University Health Board

Main duties of the job

Under the strategic direction of the Neath Cluster Lead:

  • Leading and acting as the primary point of contact for the development and implementation of Neath Cluster schemes;

  • Supporting the development and implementation of new Cluster based services.

  • Coordinating day-to-day management of any schemes identified by the Neath Cluster through its Integrated Medium Term Plan or arising from Cluster Business;

  • Presenting proposals for Cluster based schemes to the Cluster, providing explanation and clarification on all aspects;

  • Undertaking evaluation of individual Cluster based/driven schemes, evidencing outcomes and value for money;

  • Communicating and offering advice and support regarding implementation of any identified schemes;

  • Supporting Health Board colleagues to inform the wider Cluster development agenda;

  • Ensuring projects and initiatives are delivered on time, to quality standards and in a cost effective manner, adjusting plans as required;

  • Researching available or forthcoming external funding avenues in relation to the issues pertaining to the development of primary care and/or in relation to specific patient/client groups;

  • Exploring joint or match funded opportunities with other health providers or Third Sector organisations;

The Neath Cluster is made up of 8 GP practices, other primary care providers including pharmacies, opticians and dental practices, partners from social services, the voluntary sector and Swansea Bay Health Board. The Cluster aims to improve health outcomes for about 56,000 patients.

Our Practice is one of the 8 GP practices which make up Neath Cluster. The Cluster GP Lead is one of the 4 GP partners. We look after 6,200 patients offering an excellent supportive clinical and administrative working environment. Triage practice since 2015, operating Vision and askmyGP with practice pharmacist and nurse led minor illness clinics. Training practice for Specialty GP Trainees, Foundation Year doctors and Cardiff final year medical students.
