Practice Nurse - Churchwood Surgery

Job summary

To be responsible for the delivery of General Practice Nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care to the Practice population. As an autonomous practitioner you will be responsible for the care delivered, demonstrating critical thinking and skills in clinical decisionmaking in the management of patients. You will work collaboratively within the General Practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required. Good communication is essential within the Practice, with patients and with outside agencies and you will be expected to help ensure that everyone who comes into contact with the Practice holds it in high regard. Absolute discretion is required in protecting patients’ confidentiality at all times.

Main duties of the job

We are currently seeking an experienced and enthusiastic Practice Nurse to join our friendly team at Churchwood Surgery.

We are a busy, dynamic practice offering high standards of clinical care. Our list size is 7058, we have 2 partners, 4 Salaried GPs, 2 Advanced Nurse Practitioner, 2 nurses, 1 HCA and a Practice Pharmacist. Our main surgery is in Pontypool Town Centre and our dispensing branch surgery is located in Goytre. We use EMIS Web Clinical System.

We feel that this is an exciting opportunity for an experienced Practice Nurse who can deliver a broad range of primary care services including chronic diseases management, cervical cytology, travel vaccinations and childhood immunisations.

This is a part-time post – 2 -3 days per week.

Salary will be dependent upon experience, knowledge and skills.
