We are looking for a Practice Pharmacist to join our friendly and forward-thinking team at South Street Surgery, Bargoed, as a Practice Pharmacist and play a vital role in delivering high-quality, patient-centred care. Main duties of the job

As a Practice Pharmacist, you will deliver patient-centred clinical care through structured medication reviews, support the safe and effective management of long-term conditions, and provide expert advice on medicines to both patients and the wider practice team. You will manage repeat prescriptions, carry out medicines reconciliation, and identify and resolve prescribing issues, while contributing to patient safety, clinical audits, and quality improvement initiatives within the practice.

Key Responsibilities Clinical Responsibilities

  • Conduct structured medication reviews, particularly for patients with polypharmacy or complex long-term conditions.

  • Provide patient-facing and telephone consultations to address medication queries, adherence, and side effects.

  • Support the management of chronic conditions such as diabetes, hypertension, asthma, and COPD through medicines optimisation.

  • Where qualified, prescribe independently within scope of practice and in line with practice policies.

Medicines Management

  • Process and manage repeat prescriptions and medication requests in line with practice protocols.

  • Undertake medicines reconciliation following hospital discharge and outpatient correspondence.

  • Identify and resolve prescribing issues, including drug interactions, contraindications, and safety alerts.

  • Support cost-effective prescribing and adherence to local formularies and prescribing guidance.

Patient Safety and Governance

  • Monitor and implement national and local safety alerts relating to medicines.

  • Participate in clinical audit and quality improvement initiatives.

  • Support the practice in achieving quality targets and performance indicators related to prescribing and medicines management.
