Llanfyllin Group Practice is a three-surgery dispensing GP practice offering medical services to over 11,000 patients within our 600 square mile geographical area.

We are a large team but feel very much like a family with a shared ethos of delivering a high-quality service to our patients working in a stunningly beautiful rural area.

Our nursing team are integral to the way we offer care, providing a friendly, caring, and professional service to our patients, and maintaining a strong commitment along with the rest of the practice team to the continuous development of our services.

The practice is keen to promote a multi-disciplinary team approach to our patients care, we share our premises and work closely with the District Nursing team and offer a multitude of services at our practice such as a First Contact Mental Health Practitioner, a First Contact Physiotherapist, Womens health clinics, skin clinics, minor surgery, a minor injuries service and many more.

We are a training practice for GP trainees, medical students, physicians associates, pre-registration pharmacists and we train and develop our admin/reception and dispensary teams in house.

To complement our multidisciplinary team we are now recruiting to the role of Advanced Nurse Practitioner at the practice.

It is important to us that we have the right person joining us who brings a positive, committed and strong ethos of care to the role.

Main duties of the job

To act as an autonomous practitioner within your professional scope of practice providing care for patients via face to face consultations, telephone consultations, triage and home visits.

You will be able to demonstrate safe, competent clinical decision making and deliver expert care including history taking, assessment, diagnostic skills, treatment and evaluation.

The post holder will be an experienced, professional and caring person who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries including independently prescribing appropriate medication and making any necessary referrals for investigations in the appropriate manner.

About us

We work from three purpose-built sites with our sites at Llanfyllin and Four Crosses being our main hubs of care and provide medical services to over 11,000 patients. Our exceptional team comprising of 7 partners and a highly skilled clinical and non-clinical team provide high quality general practice services to meet the needs of our patient population.

We are a training practice at all levels from doctors to receptionists and have a large multidisciplinary team with a strong track record of developing services locally for our very supportive patients.

Who are we?

There are 7 partners comprising of 5 GPs, a Pharmacist and our Managing Partner as well as 4 Salaried GPs and two regular locum as needed. Most of us work part time and all have special interests. We always work together and there is always someone in the next room to mull over an issue or have a quick chat to help the day go by.

Our pharmacist provides medical expertise, supports our training of dispensary staff, and mentors pharmacy interns.

We have 4 prescribing nurse practitioners and a paramedic practitioner who support same day services, 5 practice nurses who all have areas of expertise and 3 HCAs who are always interested in developing their work and knowledge.

The clinical team could not function without the non-clinical team which is led by our Managing Partner who brings a transparent and open approach to all aspects of our large teams activities both day to day and in planning for the future.

Job responsibilities

Primary responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

b. Implement and evaluate individual specialised treatment plans for chronic disease patients

c. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

d. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. Provide wound care

f. Undertake the collection of pathological specimens

g. Request pathology services as necessary

h. Process and interpret pathology and other test results as required

i. Maintain accurate clinical records in conjunction with extant legislation

j. Ensure SNOMED CT codes are used effectively

k. Maintain chronic disease registers

l. Chaperone patients where necessary

m. Prioritise health issues and intervene appropriately

n. Support the team in dealing with clinical emergencies

o. Recognise, assess and refer patients presenting with mental health needs

p. Be an extended and supplementary prescriber, adhering to extant guidance

q. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

r. Contribute to practice targets (QIF, etc.), complying with local and regional guidance

s. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

t. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

u. Support the clinical team with all safeguarding matters in accordance with local and national policies

v. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

w. Deliver opportunistic health promotion where appropriate

x. To participate in and support an effective care management programme for the frail and housebound patients registered at the practice

Secondary responsibilities

In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:

a. Monitor and ensure the safe storage, rotation and disposal of medicaments

b. To act as a mentor to students and newly qualified staff

c. Participate in local initiatives to enhance service delivery and patient care

d. Support and participate in shared learning within the organisation

e. Develop an area of specialist interest, taking the lead within the organisation

f.Continually review clinical practices, responding to national policies and initiatives where appropriate

g.Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for registered ANP working at advanced level
