We are looking for GP with an enthusiastic approach to delivering services from a primary care setting.
The successful candidate will help us to deliver a high quality of patient care to our patient list.

The successful candidate will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
In accordance with job plan, to be mutually agreed, the successful candidate will make their selves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, reviewing pathology results, checking and signing repeat prescriptions, dealing with paperwork and correspondence in a timely fashion.
Main duties of the job
Clinical responsibilities:
In accordance with job plan, to be mutually agreed, the successful candidate will make their selves available to undertake a variety of duties including surgery consultations, reviewing pathology results, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing health education
Recording clear and contemporaneous consultation notes to agreed standards
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Sunnybank Health Centre is a GP practice covering the Cefn Fforest/Blackwood area. We have a list size of around 5700 patients. The practice delivers a high standard of patient care. We are always looking towards development of the practice, its services and ways of working in line with the changing face of the NHS. The practice offers a supportive and flexible working environment. Our team consists of GPs, Minor Illness & Practice Nurse, HCSW, Independent Prescribing Clinical Pharmacist, a well organised administration team along with a varied team of allied healthcare professionals.
