This role is for a HCSW within our nursing team. You will be seeing patients to undertake clinical duties such as phlebotomy, ECGs, blood pressures, diabetic foot checks and gathering information for their long term condition reviews. We are looking for someone who can work either full or part-time (18-35 hours).

Main duties of the job

This role is for a HCSW within our nursing team. You will be seeing patients to undertake clinical duties such as phlebotomy, ECGs, blood pressures, diabetic foot checks and gathering information for their long term condition reviews.

The post holder will:

1. Be responsible for the management of a group of patients with regard to Venepuncture, Physical Assessment, Adult Immunisations, Wound Care, Minor Surgery Assisting and supporting the clinical team in Chronic Disease Management, Health Screening, Mental Health, Mens and Womens Health, and the management of Emergency Situations.

2. Be responsible for maintaining the standards of patient care set by primary care

3. Be responsible for ensuring that all policies and procedures are observed

4. Be responsible for the support of staff i.e. Phlebotomist, learners.

5. Be responsible for providing accurate, current, comprehensive and concise records concerning the condition of the client

6. Liaise with the appropriate Clinical Specialists i.e. GPs, Practice teams and Nursing Team, in relation to individual patient requirements.

7. Demonstrate good organisational skills with responsibility for ensuring efficient and effective use of human and financial resources.

Ashgrove Medical Group covers two sites at Ashgrove Surgery, Pontypridd, and Penrhiwceiber Medical Centre, Mountain Ash. We are highly-respected training practice in the heart of the Welsh Valleys. The practice boasts an excellent and highly professional team of 7 GP Partners, 2 Clinical Pharmacists, 6 salaried GPs, 1 Physician Associates, 2 Pharm Techs, 5 highly trained nurses (inc prescriber and minor illness), 2 HCAs, 1 Phlebotomist all complimented by a supportive reception and administration team.

The range of assigned tasks will be carried out with the highest standard to support and enable registered and non-registered nursing staff to provide patient care.

Support the delivery of a comprehensive nursing service including chronic disease management.

The post holder is responsible for supporting the delivery of general practice services to the practice population, supervised and supported by registered nurses within the practice. They will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring chronic conditions, and providing health prevention and screening activities.

They will be supervised by registered nursing staff and support training of undergraduate nurses and the wider general practice team in competencies within their scope of practice.

They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing HCSW leadership as required.

The role may involve flexible working hours, and working across our two sites.

  • Specialist skills certification (Desirable)
  • Clinical supervision experience (Desirable)
  • Immunisations training (flu, shingles)
  • Skills for specialist clinical conditions eg. Inhaler technique, peak flow, doppler assessment

Experience
