Job responsibilities

Advanced Clinical Practice

1. To demonstrate advanced knowledge and skills and clinical reasoning to deal with complex cases within their specific field.

2. Independently adjust treatment plans according to need assessing appropriate management options, and initiate treatment or referral to an appropriate professional where necessary.

3. To demonstrate advanced critical thinking and analytical skills to support clinical judgment and decision making process.

4. To carry a clinical caseload providing advice and guidance to other staff on complex cases.

5. To be recognised as a highly skilled clinician within area of speciality, using a wide range of highly specialised and evidence based therapeutic interventions, treatment techniques, and modalities.

6. To carry out comprehensive assessments of patients as an autonomous practitioner, particularly those with highly complex presentations.

7. To keep comprehensive, accurate records which support the clinical reasoning process in the assessment and treatment of all patients, which meet departmental, Health Board and Professional standards.

8. To be able to carry and prioritise your own designated clinical caseload, working as an autonomous practitioner.

9. To undertake appropriate specialised interventions for patients, and where appropriate make appropriate referral to other members of the Multidisciplinary team when indicated.

10. To work with clinical specialists to develop and monitor high quality standards of care based on national guidelines, evidence based practice, clinical audit, and education opportunities.

11. To be professionally and legally accountable and responsible for all aspects of professional practice including the management of patients in your care.

12. To order, interpret and act upon medical investigations and expedite access to appropriate medical staff, for example Consultant.

13. The Practitioner will ensure they work within their scope of practice, within Health Board prescribing and other policies.

Management and Clinical Leadership

1. Leads clinical service delivery with the support and assistance of clinical specialists.

2. To work with clinical practitioners to promote high clinical standards within the service.

3. To practice and promote effective caseload management and efficient and effective use of resources within Primary care nursing

4. To be responsible for the safe and effective use of all equipment utilised and for the advice to other staff, patients and carers in relation to suitability and instruction for use.

5. To lead and chair local Clinical Groups relevant to specialist area. To identify and undertake specific projects as required in order to improve services and outcomes to patients. To develop effective communication with Team Leaders regarding prospective work programme.

6. To review current practice and evidence base, make recommendations to the Senior Management team. Through delegated authority and advanced negotiating skills, implement change to practice as appropriate.

Experience

Essential

• Clinical Experience
• You need to have a good level of experience in your chosen area of nursing, usually requiring at least two or three years of post registration clinical practice.
• Demonstrated Competence
• You'll need to demonstrate competence in advanced practice, including independent assessment, diagnosis, treatment, and prescribing.

Qualifications

Essential

• You must be a registered nurse with the Nursing and Midwifery Council NMC.
• This means completing a Bachelor of Science in Nursing, Bsc or equivalent, followed by registration with the NMC.
• Postgraduate Education.
• Master's Degree
• You need to complete a Master's degree in Advanced Clinical Practice or a similar program. These programs typically take two to three years to complete.
• Independent Prescribing Qualification
• You must also be a qualified independent prescriber. This requires specific training and assessment.
• Other Requirements
• NMC Registration and Revalidation
• You must maintain active registration with the NMC and meet their revalidation requirements.
