We are seeking a dedicated and motivatedPractice Nurseto join our friendly primary care team within a busy GP surgery in Wales. The post holder will provide high-quality, evidence-based nursing care to patients across all age groups, supporting both the treatment and prevention of illness in the community.
Working as part of a multidisciplinary team, the Practice Nurse will run nurse-led clinics and deliver services including immunisations, cervical screening, wound care, health checks, and the management of long-term conditions such as asthma, COPD, diabetes, and hypertension. They will also play a vital role in health promotion, lifestyle advice, and supporting patients to self-manage chronic disease.
The role requires strong clinical and organisational skills, as well as the ability to work independently within the framework of local and national clinical guidelines. The Practice Nurse will contribute to meetingQuality and Outcomes Frameworktargets, support practice audits, and ensure that infection control and patient safety standards are consistently upheld.
The post holder will be accountable to theGP partnersand report day-to-day to thePractice Manager, while working collaboratively with other healthcare professionals including healthcare assistants, district nurses, and community services.
Main duties of the job
The post holder plays a central role in delivering front-line healthcare alongside the GPs. Week to week, they run their own clinics as well as support doctors, typically providing services such as immunisations (childhood, flu, travel), wound care, cervical screening, chronic disease management (e.g., asthma, COPD, diabetes reviews), contraception advice, and health promotion. They also support patients with lifestyle changes, provide education on long-term conditions, and may assist in minor illness triage depending on training.
Their responsibilities extend to maintaining accurate patient records, following clinical protocols, managing stock (e.g., vaccines, dressings), ensuring infection control standards, and contributing to Quality and Outcomes Framework targets which support the practices performance.

New Chapel Street Surgery, situated off Harold Street in Pontnewydd, Cwmbran, NP44 1DU. The practice serves as a welcoming and modern NHS GP practice under the Aneurin Bevan University Health Board. The surgery caters to approximately 5,000 patients within the Cwmbran area and maintains an open registration policy for those living in its defined catchment area. It has a dedicated team including Dr Jonathan Ryder (Senior Partner and South Wales GP of the Year 2023), Dr Swapna Abraham, Dr Sian Jones, and Dr Ravi Kumar.
