We are looking for an enthusiastic and committed Clinical Summariser to join our thriving practice. Our friendly team of healthcare professionals are supported by our excellent administrative staff. We operate from 2 sites North Celynen Practice Crumlin and North Celynen Practice Newbridge providing high quality care to our patients. We are a high-achieving, well-organised practice and use EMIS Web as our clinical computer system.
Main duties of the job
1. To summarise and Read Code relevant information from a patients clinical records into the Practice computer system.
2. Undertake a variety of administrative duties to assist in the smooth running of the practice, including the provision of clerical support to clinical staff and other members of the practice team.
3. Ensuring patients medical records are up to date and of excellent standard that meets all the demands of Clinical Governance and Data Protection.

North Celynen Practice is in Caerphilly Borough County. The Practice is part of the Risca Surgery and North Celynen Partnership. We are a Teaching Practice taking in Registrars, Medical Students and Academy Staff.We have a full primary health care team which includes Advanced Nurse Practitioners, Practice Nurse Manager, experienced nurses, highly qualified Health Care Assistants as well as a highly experienced Managerial and Administrative Team.We work in purpose built, modern surgeries, Crumlin and Newbridge Crumlin. Our other Practice is in Risca. We have a large Practice population across the two sites: 13,200 Patients in Risca Surgery and 9,400 in North Celynen Practice. We are consistently high achievers. We offer patients core general medical services and provide a variety of enhanced series such as minor surgery.We work closely with District Nurses, Health Visitors and Palliative Care teams.

Review patient medical records received electronically through GP-2-GP
Culling and sorting patient records and letters in preparation for summarising
Review medical records and produce an accurate summary of the patients medical history.
Read Coding the information onto the EMIS clinical system, unless GP-2-GP where data requires verification.
Monitoring progress of notes summarising against practice targets
Offer general assistance to the Practice team and project a positive and friendly image to patients, visitors either in person or via the telephone.
Ensuring medical records are up to date and accurate.
To commit to undergoing any further training needs in order to maintain the high standard of data entry expected in patient records.
