ANP - Advanced Nurse Practitioner
*For this role we pay your NMC and MDU fees along with a competitive salary and NHS Pension.
The post holder is an experienced Advanced Nurse Practitioner who, acting within their professional boundaries, they will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. The post holder will see patients in our acute clinic and will also be expected to carry out home visits.
They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process.
They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policies and procedures. In order to work at this level NMC requirements for advanced practice must be met.
This is a permanent role with a 6 month probation period. The salary is negotiable. Hours available are between 24 and 30, and we are able to offer some flexibility on the days / sessions worked.
Main duties of the job
The ANP role is central to our service and manages much of the acute illness presented on a daily basis.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
Work with patients in order to support compliance with and adherence to prescribed treatments
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

We are looking to recruit a friendly, enthusiastic, and highly motivated Advanced Nurse Practitioner to join our team at Pendre Surgery in Mold.
We pride ourselves on being a patient focused community practice and even in these challenging times strive to offer the best personal care. We are a GP Training Practice and achieve High QAIF standards.
We are a well-established GP Practice in Mold with just over 10,000 patients. Our team consists of 4 GP Partners, 3 Salaried GPs, 2 ANPs (including this role), 2 Practice Nurses , 2 HCAs and a small Pharmacy team.
We are also supported by a health board pharmacist and have regular clinics for our patients with occupational therapists, a community psychiatric nurse and audiologists.
