SALARY:£28,800 - £45,000 depending upon experience and qualificationsBand 5-6The post holder is responsible for the delivery of general practice nursing services to the practice population across both Ashgrove Surgery, Pontypridd and Penrhiwceiber Medical Centre, Mountain Ash. They will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring chronic conditions, and providing health prevention and screening activities.
They will support the nursing manager in the supervision of non-registered nursing staff and the training of undergraduate nurses and the wider general practice team.
They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
The role will involve flexible working hours.
Main duties of the job
The post holder will:
Be responsible for the management of a group of patients with regard to Chronic Disease Management, Womens Health, Screening, Immunisations, wound care, and other treatment room duties.
Be responsible for maintaining the standards of patient care set by primary care
Be responsible for ensuring that all policies and procedures are observed
Be responsible for the support and supervision of non-registrant nursing staff i.e. HCSWs and Phlebotomist
Be responsible for providing accurate, current, comprehensive and concise records concerning the condition of the client
Liaise with the appropriate Clinical Specialists i.e. GPs, practice teams and nursing team, in relation to individual patient requirements.
Work in accordance with the Nursing and Midwifery Council Code of Conduct.
Demonstrate good organisational skills with responsibility for ensuring efficient and effective use of human and financial resources.

The practice boasts an excellent and highly professional team of 7GP Partners, 6 salaried GPs, aPhysician Associate, 4 Pharmacists, 2 Pharmacy Technicians, 5 highly trained nurses (inc prescriber and minor illness), 3 HCAs, 1 Phlebotomist all complimented by a supportive reception and administration team.
