Main duties of the job

The post holder will provide appropriate practice nursing services to the practice population and will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. Clinically the focus of the role is to provide evidence-based care for patients with acute and chronic conditions and look to improve patient health outcomes in the assessment, diagnosis, treatment and management of patients within their care working in a collaborative manner with the surgery team. The post holder will develop good working relationships with:
