Job Title: General Practice Nurse
General Practice Nurses (GPNs) form part of the practice team and share the workload and responsibility of planning and providing care, treatment and health education to patients of all ages. They provide both first contact and continuing care for the population, managing long-term conditions like chronic heart disease, diabetes, and supporting those with mental health problems, learning disabilities or dementia. GPNs deliver immunisation programmes, contraceptive services and cervical screening in line with local and national policies and guidelines and within the NMC Code of Professional Conduct. They are key members of a multi-professional team that includes doctors, pharmacists, therapists and a range of other professionals delivering person-centred care.
Main duties of the job
As part of the role development, you will be supported and given protected study/learning time one day/week for a period of 6 to 9 months and a named Supervisor/Assessor.
You will follow an individual learning and development foundation programme in line with the national Framework for General Practice Nursing (2021) which sets out the key skills and competencies of a General Practice Nurse.
The programme will follow a standardised curriculum with a blend of national and local learning alongside work-based learning and skills development. Clinically, the focus of the role is the delivery of treatment room procedures with the chance to progress and receive mentorship in developing skills to manage patients with long term chronic conditions.
As an autonomous practitioner, the nurse is responsible for the care delivered demonstrating critical thinking and skills in clinical decision-making.
Based at Ystwyth Medical Group, they will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing support to other colleagues not only within the nursing team but practice wide in all areas.
The successful candidate will have experience of working in a practice environment and be able to conduct and lead on chronic disease management of certain patient cohorts.
