Job summary

The post holder will work as key member of the practice team to provide general medical services for our registered and temporary patients.

Main duties of the job

Work within professional boundaries, demonstrating safe clinical decision-making and expert care for complex patients in a range of settings including, surgery consultations, telephone triage and general patient enquiries for urgent and non-urgent presenting cases. This will include ongoing case management, referral to other health care providers and appropriate discharge.

Job description

Job responsibilities

Role: Advanced Nurse Practitioner (General Practice)

Clinically responsible to: Clinical Practice Partners

Managerially responsible to: Practice Manager

Clinical

1. Provide a first point of contact within the practice for patients presenting with undifferentiated problems, practicing within the medical model of clinical history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.

2. Undertake referrals to other care providers, in primary, secondary and social care as deemed appropriate.

3. Instigate necessary diagnostic tests or investigations and interpret findings/reports.

4. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.

5. Manage correspondence between care providers and action accordingly

6. Screening patients for disease risk factors and early signs of illness

7. Provide support and advice to the minor illness and practice nurse team when and if patients fall outside of their management protocols.

8. As an independent prescriber: to prescribe safe, effective and appropriate medication as defined by current legislative framework e.g. the Prescribing Competency Framework, Royal Pharmaceutical Society (2016) and in line with the practice prescribing formulary or where clinically appropriate.

9. Prioritise health problems and intervene appropriately, including initiation of effective emergency care as appropriate.

10. Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills

11. Clearly communicate the health status of the patient using appropriate terminology, technology and format.

12. Provide safe, evidence-based, cost-effective, individualised patient care.

13. Work within the practice, local and national guidelines, policies and guidelines

14. Maintain accurate and contemporaneous records, utilising computer systems where appropriate abiding by the Caldicott Principles and GDPR in relation to all data handling.

5. Delegate appropriate tasks.

6. Support the delivery of health and safety guidance as agreed within the practice.

7. Support the writing of, maintenance and delivery of guidance and clinical policy and protocols within the practice.
