ROATH HOUSE SURGERYJOB TITLE:PATIENT CO-ORDINATOR
REPORTS TO: OPERATIONAL SUPPORT MANAGER/PRACTICE MANAGER
Summary
Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
Contribute to the practice team ethos and project a positive and friendly image to patients and other visitors, either in person or via the telephone. Contribute to the practice rota to provide additional cover for team members during periods of sickness and annual leave.
Job Responsibilities
Have a thorough knowledge of all Practice procedures
Work in accordance of written protocols.
Amalgamate newly registered patient's notes on receipt to the practice.
Fax and photocopy as requested.
Scanning patient correspondence onto computer records where applicable.
Provide patients with test results over the phone or in person in accordance with practice guidelines
Receiving patients and signposting patients in reception
Handling prescription requests and distribution of completed prescriptions.
Dealing with telephone queries and transferring a call to the appropriate person
Any other appropriate tasks commensurate with role
Appointments
Signposting patients to the most appropriate healthcare professional Process appointment requests.
Deal with visits requests in accordance with practice guidelines. Process ambulance/patient transport requests and update patient's computerized records.
Enter patient's details on the computer for telephone triage by the duty doctor.
Clinical systems
Registrations of new patients computer data entry and medical records where applicable.
Process patients change of address computer data and medical records (have knowledge of practice area.
Process repeat prescription request in accordance with practice guidelines. e Update patients computer records in accordance with practice guidelines.
Telephone & Building
Have working knowledge of telephone system.
Check that the call divert is on for Out of Hours Providers Answer all incoming calls
Ensure building is alarmed and secure when leaving at the end of the day
Any other tasks allocated by managers

Using personal security systems within the workplace according to practice guidelines
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
Making effective use of training to update knowledge and skills e Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
Reporting potential risks identified.
