Job summary
Llwchwr LCC is looking for an enthusiastic individual to work co-operatively with the LCC members to provide administrative and IT support to the delivery of LCC based schemes and initiatives.
The Project Support Officer will need to use their own initiative, apply a high degree of confidentiality, possess mature judgement and promote the values of partnership working.
This will be under the direction of the Llwchwr LCC Lead / Business Development & Implementation Manager to manage the collection of information in relation projects, systems, services and the IMTP (Integrated Medium-Term Plan) as required by the LCC Lead/LCC members.
Support the LCC Lead and the BDIM with the compilation of data, research, to support the writing of business cases or funding applications. Engage with wider LCC membership to develop and maintain a calendar of virtual LCC health messages
Main duties of the job
As above plus develop & maintain Vision 360 Appointments System for Cluster Services & SMS Text Service. Undertake mapping exercise of all local services within the cluster area - detailing all primary, secondary, self-referral, third sector, informal health and well-being resources and support currently available.
Build content for the Llwchwr LCC Website and Signposting Tool from the mapping content as well as detailing information about the Cluster. Map and build content foron-line Weight Management Hub (Level 1 Service) delivered through the Llwchwr LCC Website offering information, education, support and referral routes for individuals within cluster area who are seeking help to reduce their weight and live a healthier lifestyle. To build content and resources for Health Care providers with opportunities for education, referral criteria and pathways.
Ongoing maintenance of the Website, Signposting Tool & Weight Management Hub. Develop social media to enhance LCC patient engagement and conversion that will inform the current & future work and produce a quarterly Llwchwr LCC Newsletter. Provide admin support to the Weight Management Level 2 Service.
Development & maintenance of a key contacts database. Support planning of the teams work by arranging meetings & sending calendar invitations. Assist with events including booking venues etc. Managing team mailbox. Take notes of meetings as appropriate. Submitting invoices to SBUHB as required.

A Local Collaborative Cluster brings together all local services involved in health and social care and support across a geographical area, typically serving a population between 25,000 and 100,000. Working as a LCC ensures care is better co-ordinated to promote the wellbeing of individuals and communities.
Llwchwr LCC serves a patient population of approximately 50,000 and is one of the 8 LCCs in Swansea Bay University Health Board (SBUHB). LCC members include representatives from general medical services, community pharmacy, optometry and dental services, other healthcare professionals and the third sector. Working together with partners from social services and SBUHB Llwchwr LCC provides the opportunity for the development of innovative approaches to improve the health of its patients
