Job summary

We are seeking a motivated and committed Pharmacist to join our supportive, enthusiastic, and friendly team.

This is an opportunity to join a circa 17,000 patientPractice which currently operates over three sites in Treorchy and Treherbert within the North Rhondda Cluster. You will be part of a multi-disciplinary team that includes GP Partners, Salaried GPs, Advanced Nurse Practitioner, Advanced Paramedic Practitioner, Physicians Associate, Mental Health Practitioner, Pharmacists and Physiotherapists. The Practice is committed to multi-disciplinary team working in its delivery of healthcare to the community.

The practice is a training establishment for GP registrars and Foundation Pharmacists and has close links with HEIW and CTMUHB due to the GP Partners other commitments. Participation in weekly Clinical Practice Meetings would be encouraged the MDT regularly meets to discuss shared learning, audits, prescribing advice, significant event analysis and quality improvement processes and practice. The professional development of individuals is recognised and championed including opportunities to extend scope of practice or progress a specialist interest in an area.

The salary is negotiable and will be dependent on qualifications and experience. The successful applicant will be required to work across our 3 sites in Treorchy and Treherbert.

Main duties of the job

Job Summary/Job Purpose:

Supporting and progressing the development of pharmaceutical care and the medicine management agenda by ensuring safe, evidence based and cost-effective primary care prescribing within the GP practice.

  • To ensure safe, evidence based and cost-effective prescribing

  • To support the GP/cluster primary care plans

  • To contribute to the Practices prescribing systems and processes as part of the Prescribing Team.

  • To act as an Independent Prescriber or have the ability to qualify as an independent prescriber.

  • To ensure patient safety through the delivery of medication reviews and chronic disease monitoring via medicines optimisation and patient education

DUTIES/RESPONSIBILITIES:

Co-ordinating, supporting and liaising with other health professions, including hospital clinical staff on interface issues. This will include reconciliation of medicines on discharge from secondary/tertiary care.

Co-ordinating, supporting and liaising with community pharmacists on local initiatives such as the common ailments scheme.

Managing prescribing related conflict in relation to potentially difficult patients/health care professionals.

Providing and delivering educational material to members of the Practice team, support staff and members of the general public.

Providing patient education to empower them to self-care through better understanding and improved compliance of their medicines.

Developing and delivering verbal and written reports to Practice staff, other members of the primary health care team, their support staff and members of the general public at an appropriate level.

Providing medicines, therapeutic and pharmaceutical expertise and information to Health Care Professionals when required.

Educating other members of the primary healthcare team in specific therapeutic areas and the reasons behind why changes should be instigated.

Communicating and ensuring that all health care professionals in the Practice are aware of any medical alerts, national public health circulars, recalls and CSM warnings.

It is essential to have:

  • A Masters Degree in Pharmacy (MPharm) or equivalent pharmacy degree

  • A Post graduate qualification in Clinical/community Pharmacy Diploma or equivalent experience

  • Minimum of 3 years post qualification experience in hospital/community/GP practice pharmacy

  • Be a Registered member of the general pharmaceutical Council (GPhC)

  • To participate in life-long learning as part of the Continuing Professional Development programme and maintain an individual profile and portfolio of evidence supporting that profile.

  • A good general Knowledge of prescribing and pharmaceutical systems and processes, knowledge of Professional, Legal and governance issues surrounding prescribing, dispensing and primary care.

o Identifying patients for potential medication changes in specific therapeutic areas. This may include specialist groups of patients such as nursing and residential homes or visiting patients in their own home.

o Carrying out an initial assessment of the patients medication and arranging for any necessary drug monitoring.

o Inviting patients to the practice and carrying out the medication review with the patient and their carer where appropriate. All prescribed, over-the-counter and complementary medicines should be included.

o Ensuring the patients (and or carer) basic understanding of their medicines and concordance with any agreed treatment protocol

o Documenting any recommendations as a result of the medication review and discussing with a GP as appropriate.

o Implementing any agreed changes.

o Communicating any changes to the patient and/or carer, and relevant Health Care Professionals e.g. Community Pharmacist.

o Ensuring any necessary follow up as a result of the medication review e.g. checking results of blood tests, are carried out.

o Identifying and signposting patients where health and wellbeing advice or actions are agreed e.g. smoking cessation services.

Monitoring patients on high risk medicines including those on shared care arrangements to ensure clinical and safety criteria are met prior to the issuing of the prescription.
