Main duties of the job

Herefordshire and Worcestershire Health and Care NHS Foundation Trust are the lead provider of mental health and learning disability services across Herefordshire and Worcestershire supporting children, adults and older people. They work alongside community partners to support peoples mental wellbeing. They are the main provider of Integrated Community Services in Worcestershire, working alongside GPs, social care teams and the voluntary sector to deliver nursing and therapy services which keep people at home for as long as possible.

Job description

Job responsibilities

In this role:

- You will provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral and evidence-based interventions for people presenting with mental health problems

- You will work across all clinically appropriate patients, 17-years-old and over, to deliver evidence-based interventions. This does not include those accessing specialist mental health services

- Provide front line assessment to clarify the services the patients would most benefit from. This assessment may be undertaken jointly with other providers where it would be of benefit. This may be with the GP or with other statutory or non-statutory member of staff

- Design, develop and provide service information about the options available to patients

What youll need and be able to provide evidence for:

- A current professional qualification with current HCPC, Social Work England or NMC registration

- Relevant professional qualification relating to Health or Social Care e.g. in social work, nursing, OT with current HCPC or NMC registration

- RMN, BSc/DipSW, BSc OT

- Recent evidence of continuing professional development

In addition to experience, all Qualified roles will be required to provide evidence of

  • Relevant Qualifications
  • Registration with the relevant Registration Body if applicable
