Job summary
Berkshire Healthcare NHS Trust is a Community and Mental
Health trust, providing a wide range of services to people of all ages living
in Berkshire. And to do this, they employ approximately 5,000 staff who operate
from over 60 sites across Berkshire, as well as out in peoples homes and in
various community settings.
Main duties of the job
Assess and provide high standards of initial assessment
input to patients in the community.
Liaise with MDT members in treatment and management care plans
and working closely
Support OT /PT with the recommendations for patient
centred approach to achieve their rehab goals
Create, evaluate all care plans implemented and should
have a sound knowledge in discharge planning and ability to make further
referrals in to the community as required.
