Job description
Job responsibilities
Assess and monitor
the mental health and social care needs of service users and carers as part of
an integrated team, sharing information in an appropriate and secure manner
using both electronic and paper-based systems.
Conduct assessments to establish an
individuals level of need, based upon the National Eligibility Criteria.
Carry out appropriate
review or re-assessment of individuals and/or carers as required.
Respond to
safeguarding enquiries in line with the Care Act (2014) when it is deemed that
an adult with care and support needs may be at risk of abuse or neglect.
Make every effort to
ensure the service users and publics safety and well-being are met within the
Trusts risk management framework and in line with Trust policies and procedures,
identifying possible risks and alerting the team manager to any potential
serious risk situations and flagging these up on Datix.
Attend team meetings,
professional forums, mandatory training, and other training events as required.
To ensure that
continued professional development in social work is maintained updating
professional practice to maintain social work registration.
Promote models of
good practice and the role/skills of social worker and represent the profession
as required.
Participate in
regular formal supervision .
In
addition to experience, all Qualified roles will be required to provide
evidence of:
