About the Service:
CBNRT is a specialist community MDT team
treating patients with Neurological conditions in their own homes/ community
settings e.g., gyms and place of work as appropriate
Main duties of the job
To
oversee the follow-up psychosocial care plans for patients and families,
including interventions for and triaging of and patients with traumatic stress
or other mental health concerns whilst in hospital or after discharge.
To
liaise with other health and social care staff, from a range of agencies, in
the care provided to clients.
To
oversee the use of psychosocial assessment tools by medical doctors, nurses and
AHPs psychosocial
issues.

Job responsibilities
- To provide specialist psychological assessments of patients attending critical care follow-up clinic using interview, formal psychometric and other assessment methods as appropriate.
- Through thorough assessment, identify patients who require nurse-led or psychologist-led or other MDT interventions.
- To contribute to the holistic care planning of long-stay inpatients.
- To identify significant distress and/or mental health difficulties and liaise with and advise GPs and/or community mental health services regarding appropriate mental health treatment or rehabilitative care in the community, ensuring that the healthcare professionals fully comprehend the psychological aspects of critical care survivorship.
- To provide specialist psychological advice, guidance, and consultation to other professionals, to assist in the formulation, diagnosis, and treatment of clients.
