The Outreach team aims to support and engage
children, young people and families with complex needs by means of
individualised home and community treatment packages.
Main duties of the job
Reduce
children and young peoples vulnerability to mental health crisis
Maximise
children and young peoples resilience
Enable
children and young people with high levels of mental health need to be
supported in their own homes or placements
Offer
choice to children, young people and their families and carers
Avoid
inpatient admissions
Reduce
inpatient days
Offer
advice, support and supervision to systems supporting young people, such as
addiction, adoption, Childrens Care and youth offending services

Job responsibilities
Provide a quality needs-led
service as part of the multi-disciplinary team, which will include working
flexibly across a 7-day week on a shift rota between 8am and 8pm, and
participating in the on call cover as noted above. Responding promptly to
urgent and emergency requests for assessments, and meeting young people
wherever they may feel most comfortable.
Provide prompt and efficient psychosocial assessment and treatment for
children and adolescents who need additional short term support on top of
community or crisis packages by offering an alternative pathway to avoid
hospital admission, therefore treating the young person closer to home and within
the least restrictive environment.
Contribute to the
development of processes and systems within the service to accommodate
emergency and crisis situations, using flexible approaches that adapt to
individual needs.
The post holder will have Clinical Care
Coordinator responsibility for an agreed case load of patients, undertaking
CPA, including crisis and risk management.
Responsible for implementing
and co-coordinating a range of evidence based interventions for young people
and their families/carers, which will support the young persons recovery.
To work closely with developing networks with a
range of agencies including the Children, Young People and Families
Directorate, police, Addiction Services, Adoption Services, YOS and voluntary
sector providers as well as other pathways including Learning Disability
Services, and other specialist services within Oxford Health Foundation Trust.
Contribute
to multi agency care packages, assessing high risk young people and families
who are traditionally difficult to access and manage.
Demonstrates clear, effective, up to date and
accurate record keeping.

Qualifications
Essential
- Degree in Social Work or equivalent qualification
- Post qualification experience in a mental health setting, working with young people or adults with mental illness.
- Ability to travel between sites and to regional meetings
- To have ability to manage case load and act as care coordinator for clients on case list
