Job Title: Nurse Team Manager - Telephone Triage Practitioner

Main duties of the job

  • Comprehensive triage of patients referred to OOH by 111, take detailed history and formulate safe management plans
  • be able to prescribe independently and follow NICE and Eolas prescribing guidelines.
  • To always act in a professional manner. in accordance with professional body.
  • To actively engage in the teaching, mentorship and supervision of junior staff and students.
  • To contribute and support to organisational development and change as necessary.
  • To contribute to the organisational management of the service on issues such as health and safety, use of supplies and following local policy regarding incident reporting.
  • To work closely with the medical practitioners and other clinical colleagues in the provision of quality, evidence-based care to patients within the GP OOH service

  • The post holder will demonstrate a commitment to providing a quality, evidence based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.
  • To work in accordance with the NMC and HCPC rules of professional conduct and within local care pathways, policies and standards professionally and legally responsible and accountable for all aspects of his/her own professional activities and to always manage clinical risk within own caseload, whilst remaining responsible to the Senior Clinical lead for the GP OOH service.
  • To work in close partnership and liaison with primary care, acute and specialist services, Urgent Community Response (UCR), Hospital at Home, Single Point of
  • Access (SPA), Community Services, social services, mental health services and voluntary services both clinically and in relation to service development.
  • To refer to OoH clinical guidelines to support practice
  • Maintain best practices through appropriate use of research evidence and national guidelines, e.g. NICE.
  • To be actively involved in the audit of own and service outcomes as required.
  • Maintain mandatory training

The role will involve, but not be limited to you performing the above.

  • Registered General Nurse or Allied Healthcare Professional
  • Post Registration Qualification in Minorn Illness (Level 7)
  • Advanced Clinical Assessment Qualification
  • V300 Non-Medical Prescribing
  • Mentorship qualification or equivalent

Experience:-

  • Significant experience at Band 7 equivalent in assessment of minor illness
  • Competent in using Evidence Based Practice
  • Experience in Telephone triage within primary care for acute illness presentation
  • Proficient IT skills
  • Work within their own limitations and seek support from senior colleagues when required.
  • Experience working in MDT
