Job summary

Buckinghamshire Healthcare NHS Trust are currently seeking a Rehabilitation Practitioner to be an integral part of the Rapid response and intermediate care team based at Buckingham Hospital, to work within the community.

Working in Buckinghamshire, you will be part of a 6,000 person team of highly trained clinical staff, working with a Trust whose goal is to Collaborate with each other, aspire to be the best, respect everyone, and enable people to take responsibility. The care for a population of 500,000 patients across their 3 hospital sites, local bases and the community. Their stroke service is one of the best in the region, and are a centre for burns and plastics, along with a world famous National Spinal Injuries Centre.

Main duties of the job

If you are successful in your application to this post, you will be expected to support the delivery of the rehabilitation provided by the units including:

The Assistant Practitioner will provide general and specific care to Adults, and will be proficient and competent to work across professional disciplines, referring

patients for Allied Health Professional or nursing assessment and treatment or to other

agencies as required.

To be an integral member of the Rapid Response & Intermediate Care (RRIC) team,

providing prompt assessment and delivery of therapeutic treatments to patients in liaison

with senior staff.

To take responsibility for designated elements of the patient treatment protocols and

delivery of such treatments in patients own homes.

To acts as a technical resource to the Adult Community Healthcare Teams, in relations to

the provision of mobility equipment.

To document, evaluate and liaise with qualified Physiotherapists and Occupational

Therapists following interventions.

To be responsible for the completion and potential delegation of non-clinical tasks within the RRIC and ACHT teams.

Person Specification

Experience

Essential

  • Good literacy and numeracy skills
  • Have the ability to undertake training to acquire the skills appropriate for development in the post
  • Previous experience as a healthcare assistant, rehab assistant, occupational therapy or physiotherapy assistant
  • Community Intermediate Care Experience
  • Valid and current driving license and car to be able to travel to locations throughout (MILAGE PAID)
