This is a Split role capacity as a Psychology Assistant (2 days a week) and as a Home Treatment Team practitioner (3 days a week).
Main duties of the job
The South London & Maudsley NHS Trust provides a full range of
mental health services to people of Southwark, Lewisham, Lambeth, and Croydon
as well as substance misuse services for people in Bexley, Greenwich, and
Bromley. The Trust provides specialist services to people from across the UK.
It has approximately 4,800 staff and a budget of £300 million. The trust
also works closely with the Institute of Psychiatry, Psychology and
Neuroscience (IoPPN), King's College London and are part of King's Health
Partners Academic Health Sciences Centre. There are very few organisations in

You
will assess the needs of service users and plan and evaluate care using a collaborative
approach with service users, their families, and carers. You will work alongside other members of the
multi-disciplinary team to ensure effective care delivery, including the use of
evidenced based time-limited psychological interventions, risk assessments and
management, and safe discharge processes.
You
will ensure the physical health care needs of service users are appropriately
addressed and monitored. You will liaise effectively with other professionals
and services to ensure good communication is maintained and care is transferred
seamlessly.
