Location: Oxford Centre for Enablement, Windmill Road, Headington, Oxford, OX3 7HE

Ward/Dept: Oxfordshire Early Supported Discharge for Stroke Service

Hours: Full time Up to 37.5 hours per week (part-time hours will also be considered)

Shift Pattern: Monday-Friday, 8am-4pm (although flexible working arrangements can be agreed). Also, one Saturday every six weeks, if possible, in exchange for a day off in the week.

The Responsibilities

If you are successful in your application to this post, you will be expected to manage and deliver on the responsibilities outlined below as well as those not included here, call today to discuss further:

  • To deliver specialist assessment, planning and rehabilitation to a complex clinical caseload of patients who have had a stroke, supporting other clinical and non-clinical staff in the delivery of agreed plans

  • To maintain the multidisciplinary clinical records in line with Trust and professional standards

  • To be involved in multidisciplinary meetings of the whole stroke pathway

  • To work within the multidisciplinary team in partnership with the community care services to support the competence of all staff working with stroke survivors

  • To work as a clinically autonomous practitioner within the service to diagnose (where appropriate), assess, plan, and implement patient specific programmes of evidence-based rehabilitation, disability management and care, modifying plans because of clinical assessment and patient involvement

  • To work effectively across a variety of environments (including the patients home and the hospital) involving changing and demanding conditions as the need arises

  • To use a comprehensive range of verbal and non-verbal tools to communicate the diagnosis and treatment plan effectively with patients and their families and carers, within the relevant legal framework with patients who lack capacity

To be successful in applying for this role you will need:

  • Knowledge of stroke/neuro is essential.

  • Experience of working in the community is desirable.

  • Knowledge of using electronic patient records is desirable.

  • Experience of using Microsoft Office software is essential.

  • It is essential that the applicants have experience of working with neurological patients and that they are a driver with use of their own car for work.
