Job summary

Main duties of the job
- To lead on Discharge to Assess assessments.
- To provide nursing expertise and be responsible for the assessment, provision and evaluation of nursing care as required by service users in receipt of the Discharge To Assess Service -incorporating Intermediate Care at Home element of the service
- To undertake Triage of the referrals, ensuring the patient is allocated to the most appropriate service for their ongoing assessment/treatment/care.
- To prevent avoidable hospital admissions
- To facilitate earlier discharge of adults from secondary care
- To facilitate a successful return to home from bed based services
- Provide support and advice for patients, carers, relatives, and junior staff.
- To be involved in the Triage function of the Discharge To Assess Service ensuring patients are transferred to the appropriate setting for ongoing assessment /care
- To assess patients in hospital and community settings in order to ensure the appropriate care interventions are provided for the patient in order to maximise their independence
- Identify any issues that may impede the safety of the individual to be cared for in the community
- To undertake full nursing assessments and risk assessments of patients in the Intermediate Care at Home / Discharge to Assess
- To ensure the delivery of prescribed care using available resources.
- To monitor and evaluate the progress of individual goals
- To uphold a professional approach and maintain professional integrity at all times

- Evidence of Continuous Professional Development maintained in a portfolio including attendance at recent post registration courses relevant to nursing.
- District Nursing or Community Specialist Practitioner, District Nursing, qualification
- Nurse Prescriber V100/ V150

- Experience of working as part of a community nursing and multidisciplinary team.
- Experience of student supervision.
- Contributing to clinical education of less experienced staff, students, unregistered staff.
- Experience of working in a rehabilitation setting.
- Experience of working with Stroke patients, patients with dementing illness, confused patients and patients with physical limitations to mobility.
- Participation in discharge planning.
- Demonstrate responsibility for an area of work that includes all aspects of patient care
