Hours: 37.5H, Monday-Friday, 9am-5pm
Pay rate: £22.89+ Per Hour

Main duties of the job
Working within the Older Peoples Community Mental Health Team, you will offer assessment, diagnosis, treatment, and support for people over 65 who present with functional mental health illnesses. You will be working with doctors, mental health nurses, occupational therapists, psychologists, social care practitioners and support workers. The team also works closely with other services, such as social care, as well as with the voluntary sector, particularly the Alzheimers Society, to provide positive support for people using services.

To deliver a high-quality community service to Older People with mental health problems, in a multiprofessional framework.
You will act as care cocoordinator, where appropriate to patients who require the skills of a Mental Health Professional, and provide a holistic, clinical autonomous service, to enable individuals to engage in the best standards of care available.
To act as support and specialist advisor to the GP, and primary care colleagues
To assess clients according to agreed care pathways, and with appropriate assessment tools.
To develop close working relationships with voluntary / statutory services, in particular Social Services and members of the Primary Care Team, and participate in the system of integrated team working.
