The Role:
We are currently looking for a Band 5 Occupational Therapist to join our amazing team on a permanent basis.
The Northern Care Alliance NHS Group (NCA) brings together staff and services from Salford Royal NHS Foundation Trust (SRFT) and The Pennine Acute Hospitals NHS Trust (PAT).

Main duties of the job
Main Duties and Responsibilities of the role:
To carry out a holistic assessment of patients needs and develop, implement and evaluate treatment programmes for a designated caseload.
To complete referrals from Therapy Leads working alongside a fully Multi-disciplinary Team within Community Services
To work as a member of a multi-disciplinary team, coordinating discharge and taking an active role in case conference / planning meetings
To support people who have been discharged from a hospital setting by providing reablement in their own homes.
To work within the guidelines of the Therapy Team to stabilise people and help enable them to become independent.
To communicate sensitive information to patients and relevant others, relating to prognosis and condition. To discuss potential for rehabilitation / improvement and negotiate and plan future care needs for those with continued disability with empathy and reassurance.
To communicate assessment and treatment results, and information relating to condition to appropriate disciplines, and external agencies in the form of letters and reports, and through established communication networks.
To evaluate, adopt or adapt communication methods as appropriate to educational, social background or identified barriers including language disorders and cognitive problems.
To assess and gain valid informed consent and have the ability to work within a legal framework with those who lack capacity to consent to treatment.
