Job summary
Greensand Surgery are looking to welcome a pro-active LTC practice nurse to join the team on a part-time basis for 16 hours per week.The successful candidatewill provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals. Youwill be managing a range of specialities including heart failure, diabetes, CHD, and CVD.
Although we ideally would like an experienced practice nurse who is qualified and experienced in all the long-term conditions above, we would welcome applications from nurses who may only have qualifications/experience in one or two of the specialities.
Main duties of the job
Responsibilities include:
- Be proactive in chronic disease specifically CVD, HF, hypertension, asthma and diabetes, QoF and enhanced service management
- Responsible for management and running of long-term condition clinics in . diabetes, chronic heart disease and heart failure in line with NICE guidance and Quality and Outcome Framework
- Recognise signs and symptoms that may indicate undiagnosed long term conditions and utilise relevant referral pathways
- Utilise Ardens templates to complete thorough and concise annual chronic disease reviews
- Work with other clinicians and staff to provide education, support, specialist advice and guidance to colleagues
- Refer patients to other services/agencies and members of multidisciplinary teams as necessary
- Recognise signs and symptoms that may indicate undiagnosed long term conditions and utilise relevant referral pathways
- Complete medical reviews
- Alcohol screening
- Smoking cessation
- Proactive in improving and influencing patient care pathway
- Attend and participate in nurse meetings

Greensand Surgery is a semi-rural, GMS, practice in mid-Bedfordshire with just over 10,000 patients. We are a dynamic, hard-working team whose aim is to provide high quality patient centred care. The partnership is supportive and cohesive and we have a full complement of enthusiastic team members. We provide a comprehensive range of medical services for our patients as well as meeting targets for the new contract. The practice consists of three partners and four salaried GPs, a dedicated nursing team, clinical pharmacist, and admin staff.
