Job summary

Are you a passionate and motivated practice nurse with an interest in diabetes care and long-term condition management?

We are delighted to offer an exciting opportunity for a practice nurse to join our friendly and expanding team Martlesham Heath Surgery, to contribute to our ongoing commitment to delivering outstanding primary health care to our patient population. This position is offered on a full or part-time basis.

You will play a key role in delivering high-quality, evidence-based care across chronic disease management, health promotion, screening, and preventative services.

You will be supported by an experienced multidisciplinary team and encouraged to develop your skills, particularly in diabetes care, while contributing to a service that is responsive, patient-focused, and continually improving.

Main duties of the job

Providing clinical care in the treatment room, including wound care, injections, and immunisations.

Supporting the management of long-term conditions such as diabetes, asthma, and hypertension.

Performing routine health checks, screenings, and vaccinations.

Working collaboratively with the Lead Practice Nurse, GPs, and the wider multidisciplinary team.

Contributing to the development and improvement of primary care services in response to increasing demand.

Conducting routine health checks, screening, immunisations, and travel vaccinations, as well as treatment room procedures including wound care and minor procedures.

Supporting patient self-management through education, advice, and health promotion.

Working collaboratively with GPs, the Lead Practice Nurse, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.

Contributing to service development and quality improvement initiatives, helping to shape and improve the patient experience at the practice.
