Job summary

Rowhedge & University of Essex Medical Practice has an exciting opportunity for an experienced clinical pharmacist to join their growing team. The role is part-time, two days per week, Thursdays and Fridays.

This is an exciting opportunity for an enthusiastic and pro-active clinical pharmacist to join the team. We are looking for people who can support the effective delivery of patient care by utilising pharmaceutical skills to appropriately manage medical demand and ensure that patients see the right clinician.

Main duties of the job

  • Providing specialist expertise in the use of medicines and medicine information (e.g. side-effects, dosage);
  • Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice;
  • Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes;
  • Supporting practice dispensary;
  • Acting as a key contact, for all medication related queries;
  • Running clinics where medicines are the main interventions e.g. anticoagulant monitoring clinics;
  • Conducting spirometry, child immunisation, travel vaccination, diabetic foot checks and NHS health checks, as well as other tasks, as required, on completion of appropriate training;
  • Conducting care-planning, including: mental health, admission avoidance schemes and end-of-life patients.
  • Capturing relevant patient health information at all consultations including contributing to the quality outcomes framework;
  • Establishing ongoing professional relationships with individual patients;
  • Taking a central role in the clinical aspects of shared-care protocols, clinical research with medicines and liaison with specialist pharmacists;
  • Managing the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions and medicines reaching review dates;
  • Reviewing prescribing against local and national targets, and engaging with the wider prescribing team to rectify any issues.

The Colte Partnership is a GP super-partnership, formed in June 2017. It brought together 8 local GP surgeries across Colchester and Tendring, serving approx. 80,000 patients. With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county.

As well as General Practice, we offer Extended Access appointments to enable all of our patients to access medical care at evening and weekends; we work collaboratively with external agencies to provide in home services and Social Prescribing; we are working to be at the forefront of Clinical Research; and have many more exciting ventures on the horizon.

There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with on-going professional development (training support, CPD and peer support). The role will contribute to the improving quality of care of our patients across the network of practices.
