PCN Care Coordinator

Job summary

SS9 South PCN has an opportunity for a care coordinator to join our team on a part-time basis for 22.5 hours per week, across 3 days. The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.

Main duties of the job

  • Support SS9-South PCN in the delivery of high-quality primary health care projects & systems.
  • Support the Management teams & clinical director to ensure the smooth running of the PCN operations.
  • Co-ordinate attendance for the SS9-South PCN in meetings with external stakeholders, ensuring appropriate personnel attend in relation to PCN services.
  • Co-ordinate the deliverance of seasonal covid vaccination programmes including ordering, arranging mutual aid, and reporting to the ICB.
  • Oversee the deliverance of the Enhanced Access programme, to include setting rotas, sourcing and agreeing staff allocations.
  • Overseeing the usage of appointments and undertaking any required reporting to the ICB.
  • Support rota and allocations for the ECP teams & provide daily support for allocation of visits and route planning.
  • Liaise with practices own administrators regarding PCN staff availability, ensuring PCN staff rotas are aligned & available within the practices.
  • Assist in HR provision ensuring appraisals / 1-2-1s and Return to Work meetings are delivered timeously.
  • Arrange and co-ordinate mandatory training for clinicians & staff within the SS9-South PCN including practices own staff as well as PCN employed staff.
  • Organise procurement of office equipment, supplies, & services within agreed budgets for provision of the Enhanced access service.
  • Be aware of performance & quality indicators for all aspects of primary care.
  • Liaise & work with SS9-South PCN Practices on an ad-hoc project basis.
