Main duties of the job
- Conducting home visits for frail, elderly, and housebound patients, ensuring timely assessments and treatment.
- Providing urgent and routine care to patients in their homes, including chronic disease management, end-of-life care, and post-hospital discharge reviews.
- Supporting the wider practice team with urgent care, minor illness management, and long-term condition reviews within the practice.
- Working closely with GPs, community teams, and social care services to ensure seamless patient care.
